RESUMO

Objective To study the mechanism of rib fracture caused by landing on different parts of the trunk using finite element method, and to provide some new techniques and new ideas for the reconstruction of the whole process of falls from height. Methods The finite element method was used to study the rib fracture of human security model THUMS4.0 caused by landing on different parts of the trunk. Then the model was compared with actual cases and the mechanism of rib fracture caused by falls from height was analyzed from a biomechanical point of view. Results There were some differences in the stress and strain distribution as well as the rib fracture sites when different parts touched the ground. Ribs on both sides of the body were fractured when the front of the trunk touched the ground, and the fractures were mainly located in the junction of the ribs and costal cartilage and the midaxillary line area. When the right anterior part of the trunk touched the ground, rib fracture occurred first on the side that touched the ground, and rib fractures were mainly located in the area from the right midaxillary line to the posterior axillary line, and junction of ribs on both sides and costal cartilage. When the back of the trunk touched the ground, the fracture sites were mainly located on the back of the ribs on both sides. When the right posterior part of the trunk touched the ground, multiple rib fractures were likely to occur in the parts that touched the ground. The plastic strains were mainly concentrated at the fracture sites, while the von Mises stresses were not only concentrated at the fracture sites, but also at other sites. Conclusion There are some differences in rib fracture location sites and injury mechanisms when different parts of the trunk touch the ground.

Cough may be associated with complications such as syncope, urinary incontinence, pneumothorax, and less frequently, pulmonary hernia and costal fractures. Chronic cough is a cause of rib fractures and when they occur it is likely to affect more than one rib. We report a 53 year-old obese male in treatment with enalapril 10 mg for hypertension with a dry cough lasting five months. He consulted for bilateral chest pain and a Chest X ray examination showed symmetrical fractures in the seventh left and right ribs. Enalapril was discontinued, cough and pain subsided in two weeks.

INTRODUCTION: Rib fractures are the most common type of injury associated with trauma to the thorax. In this study, we investigated whether morbidity and mortality rates increased in correlation with the number of fractured ribs. MATERIALS AND METHODS: Data from 214 patients with rib fractures who applied or were referred to our clinic between January 2007 and December 2008 were retrospectively evaluated. The patients were allocated into three groups according to the number of fractures: 1) patients with an isolated rib fracture (RF1) (n = 50, 23.4 percent), 2) patients with two rib fractures (RF2) (n = 53, 24.8 percent), and 3) patients with more than two rib fractures (RF3) (n = 111, 51.9 percent). The patients were evaluated and compared according to the number of rib fractures, mean age, associated chest injuries (hemothorax, pneumothorax, and/or pulmonary contusion), and co-existing injuries to other systems. FINDINGS: The mean age of the patients was 51.5 years. The distribution of associated chest injuries was 30 percent in group RF1, 24.6 percent in group RF2, and 75.6 percent in group RF3 (p<0.05). Co-existing injuries to other systems were 24 percent in group RF1, 23.2 percent in group RF2, and 52.6 percent in group RF3 (p<0.05). Two patients (4 percent) in group RF1, 2 patients (3.8 percent) in group RF2, and 5 patients (4.5 percent) in group RF3 (total n = 9; 4.2 percent) died. CONCLUSION: Patients with any number of rib fractures should be carefully screened for co-existing injuries in other body systems and hospitalized to receive proper treatment.

PURPOSE: Massive earthquakes are harmful to humankind. This study of a historical cohort aimed to investigate the difference between earthquake-related crush thoracic traumas and thoracic traumas unrelated to earthquakes using a multi-detector Computed Tomography (CT). METHODS: We retrospectively compared an earthquake-exposed cohort of 215 thoracic trauma crush victims of the Sichuan earthquake to a cohort of 215 non-earthquake-related thoracic trauma patients, focusing on the lesions and coexisting injuries to the thoracic cage and the pulmonary parenchyma and pleura using a multi-detector CT. RESULTS: The incidence of rib fracture was elevated in the earthquake-exposed cohort (143 vs. 66 patients in the non-earthquake-exposed cohort, Risk Ratio (RR) = 2.2; p<0.001). Among these patients, those with more than 3 fractured ribs (106/143 vs. 41/66 patients, RR=1.2; p<0.05) or flail chest (45/143 vs. 11/66 patients, RR=1.9; p<0.05) were more frequently seen in the earthquake cohort. Earthquake-related crush injuries more frequently resulted in bilateral rib fractures (66/143 vs. 18/66 patients, RR= 1.7; p<0.01). Additionally, the incidence of non-rib fracture was higher in the earthquake cohort (85 vs. 60 patients, RR= 1.4; p<0.01). Pulmonary parenchymal and pleural injuries were more frequently seen in earthquake-related crush injuries (117 vs. 80 patients, RR=1.5 for parenchymal and 146 vs. 74 patients, RR = 2.0 for pleural injuries; p<0.001). Non-rib fractures, pulmonary parenchymal and pleural injuries had significant positive correlation with rib fractures in these two cohorts. CONCLUSIONS: Thoracic crush traumas resulting from the earthquake were life threatening with a high incidence of bony thoracic fractures. The ribs were frequently involved in bilateral and severe types of fractures, which were accompanied by non-rib fractures, pulmonary parenchymal and pleural injuries.

La tos es un frecuente motivo de consulta en la práctica ambulatoria. Aunque generalmente es autolimitada, cuando es crónica e intensa puede ser causa de complicaciones como síncope, neumotórax o más raramente fracturas costales. Presentamos un paciente con fracturas costales múltiples inducidas por la tos. El diagnóstico fue confirmado por un centellograma solicitado luego de la sospecha clínica debida a la intensidad y persistencia del dolor y a pesar de no mostrar alteraciones en las radiografías convencionales. Un centellograma de control a los 5 meses mostró desaparición de los focos hipercaptantes. Las fracturas costales múltiples son una complicación infrecuente de la tos que debería considerarse cuando el dolor torácico es intenso y persistente aun con radiografía de tórax o parrilla costal normal.


Multiple rib fractures associated with cough.


Cough is a common symptom in ambulatory practice and in general does not require specific treatment. When the cough is chronic it can provoke complications such as syncope, neumothorax or, rarely, rib fractures. We report a case of a patient with multiple rib fractures caused by cough. The diagnosis was performed by scintigraphy but fractures were not found in the x-rays. Five months later, a scintigraphy showed disappearance of the increased uptake areas. Multiple rib fractures are a cough complication which should be considered when the patient’s pain is persistent even if there is no x-ray evidence.

This paper reports on a unique, previously unreported, successful outcome in the case of a patient with focal osteolytic lesions of the ribs as a first sign of osteoporosis. The lesions were detected by chance after acute cough-induced rib fractures were seen on plain chest radiographs. The diagnosis had to be approached as a diagnosis of exclusion since known causes of the osteolytic process had to be eliminated. The authors describe multiple focal osteolytic lesions with rib fractures appearing in a pattern that could be confused with metastases. Laboratory results were normal. Final diagnosis was based on plain radiography, bone scan and bone densitometry. Pharmacomedical treatments for osteoporosis were applied. The patient was observed between the year 2000 and 2005. Five years later, radiological and bone scintigraphy revealed resolution of the lesion. We conclude that osteoporosis should be included in the differential diagnosis of asymptomatic focal osteolysis of the ribs with rib fractures as a complication of acute cough. The case suggests that focal osteolytic lesions of the ribs may regress over time and become scintigraphically inactive.


Este trabajo presenta la evolución clínica exitosa, única, y no reportada con anterioridad, del caso de un paciente con lesiones osteolíticas focales en las costillas, como primera señal de osteoporosis. Las lesiones fueron detectadas por casualidad, luego que en unas radiografías simples de tórax, se observaran fracturas de las costillas inducidas por tos aguda. El diagnóstico tuvo que ser abordado como diagnóstico de exclusión, ya que las causas conocidas del proceso osteolítico tenían que ser descartadas. Los autores describen las lesiones osteolíticas focales múltiples con fracturas de costilla presentes en un patrón que podría confundirse con metástasis. Los resultados de laboratorio fueron normales. El diagnóstico final se basó en la radiografía simple, escaneo óseo y densitometría ósea. Se aplicaron tratamientos fármaco-médicos para la osteoporosis. El paciente estuvo sujeto a observación entre el año 2000 y el 2005. Cinco años más tarde, la centellografía ósea y radiológica reveló la resolución de la lesión. Concluimos que la osteoporosis debía incluirse en el diagnóstico diferencial de la osteólisis focal asintomática de las costillas con fracturas de costillas, como complicación de la tos aguda. El caso sugiere que las lesiones osteolíticas focales de las costillas pueden experimentar una regresión con el tiempo y hacerse centellográficamente inactivas.

O trauma é uma doença que afeta predominantemente os jovens, sendo a principal causa de morte nas primeiras três décadas de vida. O tórax instável é observado em aproximadamente 10 por cento dos casos de trauma de tórax, com mortalidade de 10-15 por cento. O caso clínico apresentado refere-se a uma paciente, vítima de acidente automobilístico, com trauma de tórax, ocasionando hemopneumotórax e fratura de múltiplos arcos costais, dor de difícil controle e deformidade torácica. São discutidas as opções terapêuticas. No presente caso, foi realizada fixação cirúrgica, com bom resultado.


Trauma primarily affects young people and is the leading cause of death in the first three decades of life. Flail chest is observed in approximately 10 percent of all patients with severe chest trauma, and the mortality rate among such patients is 10-15 percent. We report herein the case of a car accident victim with chest trauma causing hemopneumothorax and multiple rib fractures, intense pain and deformity of the chest wall. Surgical stabilization was performed, with good results. Therapeutic options are also discussed.

El MI es desafortunadamente frecuente y nuestro país no es la excepción. Se presenta en todos los niveles socioeconómicos, religiones, culturas y representa un desafío diagnóstico para el clínico y para el radiólogo. El rol de la radiología es crucial para su detección. Los padres son los principales causantes de MI y generalmente se excusan en accidentes comunes del hogar, por lo que debe tenerse un alto grado de sospecha. Los hallazgos radiológicos deben correlacionarse con los mecanismos del traumatismo e injuria entregados en la historia clínica. Las lesiones óseas se pueden producir en cualquier parte del cuerpo, pero hay cierto tipo de lesiones y localizaciones (fracturas metafisiarias, fracturas costales posteriores, fractura primera costilla, escápula, apófisis espinosas, esternales y fracturas en diferentes etapa de cicatrización) que deben aumentar la sospecha diagnóstica. Ante la sospecha de MI debe solicitarse un estudio completo de imágenes, incluyendo fundamentalmente estudio radiológico del esqueleto y TAC de cerebro o del resto del cuerpo si es necesario. Sólo con una alta sospecha y un adecuado estudio, será posible detectar y proteger a éstos niños víctimas de maltrato.

35 cases of blunt cardiac trauma following vehicular accidents brought for autopsy to the morgue of Regional Institute of Medical Sciences, Imphal have been studied to find out types of cardiac injuries, their association with sternal and rib fractures, mechanism of causation, risk factors, etc. It was observed that 48.57% of the cases with blunt cardiac trauma had associated sternal and rib fractures. Maximum number (60%) of the cardiac rupture was seen in run-over cases. 40% of the cases had injury to the right ventricle. All the injuries were located on the anterior surface of the heart. In one (2.86%) case, laceration of the right atrium without any external injury of the chest region was observed. In assessing blunt cardiac trauma victims in vehicular accidents, knowledge about the commonest sites, types and degrees of injuries as has been highlighted in the present study will be of great help in a timely intervention.
